Please note that I have transitioned to online intake forms that will be sent to you at the time of booking your initial visit. If you do not have access to a computer or other device to complete online intake forms, we have tablets available at the office. The forms take 15-20 minutes to fill out and should be completed in advance of your appointment, to avoid delays in starting the initial visit.
